| You can be
referred from your Primary Care Physician, Neurosurgeon,
Orthopedic Surgeon, Neurologist, etc. Bring your
medical records (all scans and reports pertaining to your
complaint) to the initial evaluation. Or you may log
onto the Patient Portal to
complete most of your basic information electronically, if you
choose this option, please call the office for a user name
and password. Based on your
medical history and physical examination, we can determine if
our treatments will benefit you. After your
evaluation, a treatment plan will be formulated and
implemented.
